What is a report?

• Used to indicate that something has been stated, although one cannot confirm its accuracy.

• Present oneself formally as having arrived at a particular place or as ready to do something.

• Give a spoken or written account of something that one has observed, heard, done, or investigated.

• Cover an event or subject as a journalist or a reporter.

The process of recording worksite events, including near misses, injuries, and accidents.

This reports is to provide information in an organized, objective way, without analysis or recommendations; in other words, to report the facts.

What should be present at the beginning of an Incident Report?

• Your name and ID Number

• The date, the time and location of incident

• Names of other persents who were present

An Information Report can be used by:

• An authority to send information such as status report for a parent.

• A worker to report an incident he/ she has witnessed;

• An authority to create a report of an incident

Poorly written Incident Reports are?

• Short and NOT to the point

• Judgemental towards others

• Missing information (Ex., Badge numbers)
