A gas leak caused a major explosion and fire at Venezuela's Amuay refinery, resulting in 41 deaths and over 80 injuries. Most victims were National Guard members. President Hugo Chávez ordered an investigation and declared three days of national mourning. Operations at the refinery will not resume until the fire is completely extinguished.
