• It should be filled out as soon as possible with whatever writing implement you can find, colour and type doesn’t matter.

• You only need to fill out an incident report form if an ambulance has been called or it is required later on.

• The incident report form should be filled out as soon as possible after the incident with a blue or black pen and all areas are to be filled out.

• Any areas of the incident report form that don't apply should be left blank.
